Wai'alae Charter School at Winners' Camp

Welcome! This form is for Wai'alae Charter students and chaperones attending the program at Winners’ Camp!


Please submit one form per participant. Parents/guardians must complete this form for minors.


For ropes course safety, we ask participants to wear pants/long shorts and covered shoes.
